PUBH1407 –Health Care in Australia Assessment 2: Patient case study Assessment preamble In order to provide high-quality patient care, each member of a multidisciplinary team needs to examine and understand many different aspects about the patient, including their medical history, social history and current treatment, as well as their viewpoint, goals and treatment preferences. Your tasks are designed to provide you with this experience. Assessment instructions Choose one of the three patient case studies (i.e. either Dev, Maree or Soula). Answer the following questions, basing your answers on your chosen case study. 1. Research unfamiliar terms: understanding all aspects of your patient and their care is very important. For this question: a. Identify and list any unfamiliar terms, conditions, medications or information that you are unfamiliar with. b. Research the items on your list and describe what they are/what they mean based on your research. Make sure you reference your answer in APA style (see lecture notes from week 3 to assist you). 2. The multidisciplinary team: a. Which healthcare model(s) do multidisciplinary teams operate under? Which model is associated with best practice management of multidisciplinary health care in Australia? b. Describe cohesive teamwork practices, and why working collaboratively in a team assists with effective decision-making in the health care system c. Identify which multidisciplinary team member you will represent, and briefly outline your role in treating the patient as this health professional d. List the other health professionals that would be involved in your patient’s care, and briefly outline the role each would play 3. Your patient: Describe how your patient’s condition will affect their ability to complete functional tasks (e.g. preparing food, personal care, walking to the bathroom) 4. Past medical history:What is your patient’s medical history and/or co-morbidities? Will they impact your approach to managing this patient’s care as their treating health professional? Why or why not? 5. Social history:What is your patient’s social situation? Describe the social concerns which may impact on how your patient can return home 6. Goals: Consider if your chosen patient has any unique recovery goals, and list them briefly 7. Patient viewpoint: Based on what you know of the case, what do you think the patient’s viewpoint might be regarding their injury/disease/treatment 8. As per the reading of the case study there are some unfamiliar terms in the scenario and that are resection, heartburn, nausea and osteopenia (Morton et al., 2017). These are the particular terms and conditions with which I am unfamiliar since I do not have any former knowledge on these aspects and thereby have no such idea about the same.  An analysis of each of the term or condition is provided here. Osteopenia is related to bone and this is a condition where the mineral and the protein content of the bone tissue get reduced. Nausea refers to the feeling of vomiting and an uncomforting aspect in the upper stomach (Morton et al., 2017). Heartburn is a burning feeling in the chest and is also known as a form of indigestion. Resection is a condition that refers to removal of any organ or body part.  Get solution
